If you submit a form, we may receive your name, role, organization, phone, email, county, and non-PHI notes so we can respond to your inquiry.
Secure referral path
If a case fits Palm’s service model, we may provide a separate secure referral workflow for patient-specific information.
Analytics
This static site does not include third-party tracking scripts by default in this package. If analytics are added later, review the configuration for healthcare privacy risk.
