Condition focus

Pressure Injuries

Pressure-injury support for homebound and facility patients where surfaces, positioning, dressings, and care-team communication matter.

Common care gaps

Pressure injuries often worsen when turning schedules, support surfaces, nutrition, and dressing changes are not coordinated.

Palm approach

The model emphasizes setting-aware assessment, facility/home communication, support-surface review, dressing-plan clarity, and escalation when deterioration or infection is suspected.

When to escalate

Rapidly worsening redness, fever, severe pain, uncontrolled drainage, black tissue, suspected ischemia, or systemic symptoms may require urgent physician, hospital, or emergency evaluation.

How to start

Use the referral conversation form if you are a care partner. Do not include patient-identifying health information in the public form. We will confirm whether a secure referral path is appropriate.
