The model emphasizes setting-aware assessment, facility/home communication, support-surface review, dressing-plan clarity, and escalation when deterioration or infection is suspected.
When to escalate
Rapidly worsening redness, fever, severe pain, uncontrolled drainage, black tissue, suspected ischemia, or systemic symptoms may require urgent physician, hospital, or emergency evaluation.
