Palm Wound Care fills that gap with a wound visit in the first days after discharge — at home, in an ALF, or at a SNF — and coordinates with the discharging hospitalist or surgeon. If a home health referral was placed at discharge, we coordinate with that agency. Family education on signs that warrant a call back to the surgeon is part of every visit.
